Layout Request!

Looking to get a new page layout for my account. I want to use the same horse image that is at the top, that's not going to be changed. 

I'm looking to get a tabbed layout so I can sort information into its own pages. I would need a few textboxes on each page. 

I would like to use the same colour theme for this entire thing..
Offering up to $120,000 for it
