Layout

So, my layout has been acting wonky, and need a new one cause it is not going back to normal. So, i just want one with a couple of text box, scrollies, Maybe a header above the tab thingies? Just whatever  you can do :)  Not looking to spend my whole bank account, so, nothing outrageous!
