Simple Layout/Image

I am looking for a simple layout for my page, all I need is one of my images edited to have my name, maybe a nice border, and then two text boxes without the scroll visible though.  I can pay with 2-3 credits or some EV Cash, not sure what going rates are for things but if someone could help I would appreciate it :) :)
